Thank you for your interest in enrolling your child at New Beginnings Preschool Center! We're a play-based Christian learning environment in Milton, Florida, and we'd love to have your family join ours.
This form takes about 10–15 minutes. Before you begin, have the following handy:
- Your child's date of birth
- Health insurance information (provider and policy number)
- Documents to upload: immunization records, school physical, birth certificate (images or PDFs, max 5 MB each)
- Contact information for at least one parent or guardian
